SPRAVATO® Cost and Insurance in Virginia: A Plain-Language Guide From TRIP

Article Overview

SPRAVATO® is one of the few treatments in psychiatry that people ask about by name, and the second question is always about money. This guide walks through what SPRAVATO® costs in practice, how insurance handles it, what the manufacturer's savings program does and does not cover, and how TRIP, our interventional psychiatry service line in Chesapeake, manages the process for you.

First, what you are paying for

SPRAVATO® (esketamine) is a nasal spray approved by the FDA for adults with treatment-resistant depression and for depressive symptoms in adults with major depressive disorder who have acute suicidal thoughts or actions. Since January 2025 it is also approved as a stand-alone treatment for treatment-resistant depression, not only alongside an oral antidepressant. It is available only through an FDA safety program called a REMS, which means every dose is given in a certified clinic with at least two hours of monitoring afterward. It is never dispensed to take home.

So each treatment session has two parts with two costs: the medication itself, and the clinic visit where you receive it and are monitored. Insurance treats those two parts differently, which is the source of most of the confusion.

The treatment schedule, because it drives the math

The standard course is twice a week for the first four weeks (the induction phase), then once a week for the next four weeks, then once a week or once every two weeks for as long as it keeps working. That is roughly twelve sessions in the first two months. Anyone quoting you a "cost of SPRAVATO®" without saying per session or per month is not giving you a useful number.

Does insurance cover SPRAVATO®?

Most commercial plans, Medicare, and many Medicaid plans cover SPRAVATO® for adults who meet the FDA criteria, and nearly all of them require prior authorization first. TRIP is in-network with nearly every major plan, with TRICARE the notable exception. Insurers typically ask for documentation that you have major depressive disorder and that you have had an inadequate response to at least two antidepressants at adequate doses and durations; some still ask that you continue an oral antidepressant during treatment, though the monotherapy approval is slowly changing those policies. TRIP gathers the records, submits the request, and follows up with the insurer. Approvals often take one to two weeks. If a request is denied, we appeal with additional documentation, and denials are frequently overturned.

Your out-of-pocket cost then depends on your deductible, your copay or coinsurance, and whether the medication is billed under your medical benefit or your pharmacy benefit. Plans differ, and this is exactly why TRIP verifies your benefits and tells you what to expect before your first session rather than after.

The SPRAVATO withMe savings program

The manufacturer's program can reduce the medication portion dramatically for people with commercial insurance. Eligible clients pay as little as $10 per treatment for the medication, up to an annual maximum benefit of $8,150 per calendar year. Three limits matter:

  • It is for commercially insured clients only. It is not available if you are on Medicare, Medicaid, TRICARE, or another government plan.
  • It covers the medication cost only. The in-office observation and monitoring fee is billed separately to your insurance and is your responsibility under your plan's usual cost-sharing.
  • Terms change. Our staff checks current eligibility and helps you enroll before you start.

Medicare, Medicaid, and TRICARE

SPRAVATO® is generally covered by Medicare under the medical benefit, with the usual Part B cost-sharing, and by many Virginia Medicaid plans, typically with prior authorization. TRIP is in-network with nearly every major plan, so for most people the benefits check is a formality. Because the manufacturer's savings card cannot be used with government plans, the out-of-pocket picture is different from a commercial plan, and we walk through it with you individually. Do not assume it is unaffordable before we have checked.

The one exception is TRICARE. TRIP does not participate in the TRICARE network, and TRICARE does not use the manufacturer's savings card either. If you are a TRICARE beneficiary, call us before assuming SPRAVATO® is out of reach; there are sometimes referral or out-of-network paths worth exploring, and we will tell you plainly what is and is not possible.

What if I have no insurance, or coverage is denied?

Self-pay SPRAVATO® is expensive because the medication is expensive, and I will not pretend otherwise. If coverage is denied after appeal, we talk honestly about alternatives: optimizing oral medication, augmentation strategies, TMS through another provider, or in some cases racemic ketamine, which is off-label but less costly. My job is to find something that works within what you can actually sustain.

Hidden costs people forget

  • Transportation. You cannot drive on treatment days. Plan for a ride for every session, which in the first month is eight sessions.
  • Time. Each visit is a little over two hours in the clinic. Many clients schedule late-day sessions to protect their workday.
  • Your oral antidepressant. If you continue one, it is a separate prescription and copay.

Frequently asked questions

I already see a psychiatric provider. Can I get SPRAVATO® at TRIP and keep my prescriber? Yes. We coordinate with your existing provider, with your permission, and you come to TRIP only for treatment sessions.

Does the $300 SCP evaluation fee apply to SPRAVATO®? No. SPRAVATO® care runs through TRIP and is billed to your insurance. If you need a psychiatric evaluation and do not have a prescriber, South Chesapeake Psychiatry can provide it separately.
